Forms
Forms can be used to make a change to your super or retirement account(s), access your money, make contributions and otherwise manage your super. Some are online and others printable PDF.
Brochures and fact sheets
Brochures contain information about accounts and membership types, insurance, investments and accessing super. Fact sheets provide information about a single topic.

Other useful forms
Transfer / roll in all or part of your super balance from another fund to your GESB account.
Apply to transfer existing insurance from another fund or insurer to your GESB Super or West State Super account.
Apply to open one of our Retirement Income Pension accounts, either an RI Allocated Pension or Transition to Retirement Pension.
