How to complete Centrelink’s ‘Details of income stream product’ form (SA330)

Centrelink’s ‘Details of income stream product’ form (SA330) is used by Centrelink to calculate your eligibility to receive a payment. You may be asked to provide this form when applying for a government pension or providing information on your income.

GESB is authorised by Centrelink to provide you with a Centrelink schedule, which can be used instead of the SA330 form.

This means if you have a Centrelink schedule from us, you do not need to complete the SA330 form or fill out the ‘Authorisation’ section on page 10.

To receive a Centrelink schedule, follow the steps below:

  1. Contact us on 13 43 72 or via Live chat to request a Centrelink schedule.
  2. Once received, you will need to provide the schedule to Centrelink. You can do this through your online Centrelink account, by post to Services Australia, or in-person at one of Centrelink’s service centres

Please note: a separate schedule will be needed for each allocated pension or term allocated pension you have. You will also need to request an updated Centrelink schedule from us if you change your pension amount at any time.
